Never had anything like that in any of my saloons once, never mind several times.

They sometimes end up a bit skew-whiff, but pretty straightforward to sort out...

Beats me - it's not as if the belts are short. I've tried to re-enact it and you'd need to be really careless/heavy-handed... as some people apparently are.
