Andrew Adams from Bonymaen in Swansea, was referred by the high street chemist twice as Suicide Bomber in the missive as well as on the Advantage Card that came with it.

The company, which has now apologised to Mr Adams, wrote: "Dear Dr Suicide Bomber,It’s time to treat yourself. Your shiny new advantage card is here. Isn’t it great when you get something new? Well, here’s your new Boots advantage card, so welcome to the best excuse you could have to go shopping."

Mr Adams, 63, said he was baffled as to why he had been called Dr Suicide Bomber.

He said: “Most of my life I have spent driving taxis and lorries. I’ve never been a doctor. Unless you class me as a doctor of life. And I’m definitely not a suicide bomber. I intend to live a long time to see my daughter grow up and get married."

The card Mr Adams received through the post

Mr Adams said was surprised no-one picked up on the gaffe before it was posted.

"I’m utterly amazed that it ever saw the light of day. I’m shocked that no-one in the Boots organisation picked up on this and thought: ‘This will have repercussions’.”

A Boots spokesman said: "We are very sorry for any upset that Mr Adams has experienced and we are liaising directly with him.

"As soon as we were aware of this incident, we immediately triggered a full and detailed investigation into how this appalling hoax was able to occur. This has included a comprehensive audit and a review of the systems in place to prevent such behaviours taking place again.

"We have technology in place to prevent offensive terminology and potential fraudulent names being used."

The letter arrived "out of the blue" on Monday morning at Mr Adams’ home.

He said: "I thought, ‘This is someone’s idea of a sick joke or something has happened that has slipped through the system. Boots assured me this was not normal practice and apologised and said they would investigate.

"The next person I phoned was my MP because I think she should ask questions about this. I have got quite a strong character and don’t get upset over trivial things. But what I thought was that the postman might have got the wrong idea and said: ‘I think there is a suicide bomber in Caerphilly Avenue’.

"They could have been busting my door down. Or this could have gone through to someone who has got a weak heart, and who knows what could have happened?

"I can only assume that someone has played a practical joke or someone has got something against me and either phoned up Boots or gone on line and had it sent to me."

Swansea East MP Sian James dubbed the letter "a very regrettable incident."

She said: "Boots reacted swiftly once they were made aware of the situation and are putting measures in place to make sure it doesn’t happen again."

Mr Adams, who lives at home with son Kevin, 22, and daughter Chantelle, 13, said he does not shop at Boots much.

He said: "Apparently whatever money I spend at Boots I can get points with on this card that you can exchange for goods. I’m definitely not going to use it though. I might get funny looks. And I might get outside Boots to find a police officer waiting for me."

If that is a practical joke they had better watch out the courts take a dim view of such things and a company like boots will prosecute as a face saving excercise
