I believe there are two differences.
Some vehicles are converted to M1 specification and then tested as such. The certification is the evidence of type approval and that the conversion is fully tested.
Other converters use parts that have been tested to M1 specification individually but not actually whilst fixed to a vehicle. These vehicles are therefore not certificated.
The former is generally more expensive than the latter and so a lot of people would choose the cheaper option if it was not a local requirement to produce a certificate.
In my opinion all vehicles should be certified before they are considered, but unfortunately people claim that their vehicles are something other than they are. Professional converters may convert a Peugeot Expert (for example) but it doesn't make that vehicle an E7.
Its about raising standards ................ and making sure a specific vehicle reaches the standard is something that should be properly considered.
