Ok recieved medical form from the council, and as Stinky Pete pointed out, yes it does mention medical records, dunno why, as DVLA do not have this requierment for a group 2 medical.
Any way what it actually says is "The medical examination has been carried out with refernce to the patients current medical records"
It does not mention what doctor you go to. it does ask for your doctors details only.
The way i would understand this is, if i have a copy of my current medical records i can freely go any were i want to take my medical, would any one agree with this

Clearly they just want your doctors details, it is for you to decide who your doctor is.

my post did say it didnt mention med records, but it does.
Any way cheers for the answer Sussex, I spoke to licensing about this last week but they still havnt got back to me with an answer.
The way i read it, its open to go anywere, as long as were i go can view my medical records. this is York council but intent to go to Leeds for the medical, £55 to have 2 forms filled in compared to £180 by my own doctor.

Well if they require your own docs name give them it, but you could even be seen by a locum if your own one is busy
