When MUST a hack (in England) use the meter, and when can he NOT use it?

and is there any link/reg/case law?

Depends on your local rules.

There are some areas that require you to have hackney meters.

In most area the rules are the meter must go on for all in area work, and on in all other work unless an agreement is made with punter prior to the start of the job.

Otherwise in area start and finish, on meter, outside start or finish, on meter unless agreed price before departure.

but if any or all of a trip is outside the LA the applicable LA's meter rates would be different and they wouldnt have jurisdiction to enforce a rate not theirs in a vehicle they dont plate

The applicable rate would be the rate set by the LA which licenced the cab, not the LA who's area you happen to be in.
