 Post subject: dbs form.

On the DBS application form under section 4 the question is asked "have you ever been known by any other names." Then sections 5 - 13 ask for the details.
My wife has filled this in with her maiden name and then her first married name and finally her current name. The licensing officer who has checked the form said that it would probably get sent back because the names should have started with her current name and gone backwards to her maiden name. I can see nothing on the form that says the names must be in any particular order just that there should be no breaks in the dates.
Anyone else had a problem with this?
Also When I did my DBS 3 years ago I signed up to the annual update service. The licensing officer has no record of me supplying the details to the council for them to check my report although I have sent them a copy of my original email with the details. I also checked on the DBS site if my report had ever been checked and it actually states that it was checked last September by the person who has no knowlege of my details. She told my wife this morning that someone must have used her details to fraudulently check my DBS. How likely is this?

I assume you are referring to the check on my DBS.
I think it has something to do with the last meeting that drivers had with the council. I asked the question "when we provide our updat details, does that mean that the council are free to use them when they want or do they need to still ask our permission before checking?" Now as far as I was concerned, by providing my details I was authorising the council to use them. However the head of department stated that our details would not be used without our express permission on each occasion. because last year my permission was not sought I think the LO is worried.

There's been a serious offence committed here, your sorry she is saying someone else has used her password? So I would report it to the police. As for them not seeing your dbs that's fine just take it down and show them.

I know, but they can't remember, apparently someone has hacked there system, you have the original so just as easy to show them again.

That is the point. The person whose name appears as having checked my file says that they didn't check it. They don't know who did.

This why im hesitant to even produce my DBS to COUNTY (not LA) who have already used the update to check a DBS they havent seen yet and want my DBS produced to any staff member at a location out of the office....noooooooo

The thing is, the update screen doesn't actually have any information from my DBS report. It just say that there have been no changes to the report.

Thats quite correct

You present the DBS certificate once and it has a reference number

if you commit a crime (and get done) it will appear as a change next time they look on the update screen
