As you well know the PH fare is set by the PH operator......its normally set lower than HC fares which are usually used as a standard.
There's no calculations.....provided the fare in a PH is chaeper than a HC.
The driver of a PHV is the last thing on the mind of any operator.

Amen to that, hence why there are so many foreign drivers who are on a 3 week rota of arrive drive and leave again, PH has become a slave ship, as you and Mark mention the op doesn't give a fig and will set it as low as he can in an attempt to glean more work, the funny thing is though there is nothing cheap about what the ask from the driver though in radio and uniform charges, then the driver shouldered discount on top of what is already almost a bus fare, they basically have the ethics of an African Warlord tbh and are ruining the who thing as a job for those that want to escape 9 to 5 jobs,
When I started in the 80's I worked open office and the difference between PH and HC fares was 10p, in the early 90's some bright spark decided she couldn't cope at busy times and a shift system was introduced which ensured you had to be there, now there is no open office and the fare difference has widened to £1.50 on average for a 2-3 mile fare,
They will kill it altogether eventually, as more and more foreign drivers get mugged off with the dream lie and overcharge to the level where it should be anyway the public will simply not take a cab anymore, then you will be back to drink driving on a mass scale as well,
Not good, and if you look properly the majority of issues the LC is having to review are the direct result of PH not playing the game, I'd like to see the company get fined rather than the driver for flagging, you'd soon see it all stop then and rules adheared to.
